Do I have to set up an account before I order and ship?

 

Yes. Establishing an account is easy; simply read our Terms of Use Letter which explains what to expect from our donor program and what we expect from you when purchasing and utilizing our services.  The final page of this document must be completed and returned prior to shipment of any donor specimens.  Feel free to contact Client Services for assistance if needed.

I am unsure if my physician has an account, what should I do?

 

Cryogenic Laboratories, Inc. (CLI) requires that all patients are working with a Licensed Physician/Medical Professional.  Our Client Services department can verify if your physician already has an established account by calling 1-800-466-2796.  If an account has not already been established, a physician account set up form can be faxed directly to your physician's office by providing their fax number to one of our Client Services Specialists.  If you do not already have a licensed physician working with you, you can contact Client Services and we will be able to provide you a list of physicians in your area who specialize in reproductive medicine.

Who will the shipment be sent to?  Can I perform a procedure at home?

 

Typically shipments are sent directly to your physician's or medical professional's office.  Once your health care provider has established an account, we can arrange for donor sperm to be shipped to just about any location; however, you will need to have authorization from your physician/medical professional if it is to be shipped to your home. Authorization is achieved by having the physician's office complete a Home Consent Form we fax to their office for completion. This form must be completed by your physician and faxed back to Client Services before any order is placed. Instructions for performing the procedure at home as well as any necessary equipment you might need must be obtained from your physician's office.

Do I place my order with you or do I contact my physician to place the order?


Most of our accounts are set up for patients to contact us with their payment information. Your health care provider should inform you how they wish for you to place your order. If you are paying for the order please contact us directly by calling Client Services or by ordering online. If this is your first order we do suggest you place your order over the phone so that you become familiar with the ordering process. If your physician is billing his or her office for the order then someone from his or her office will place the order for you.

When do I need to place my order?


We recommend placing your order as soon as you know the date your physician wants the units delivered to ensure donor availability; preferably one week prior to your scheduled procedure. You can place your order and establish a shipping date up to thirty days in advance. If your procedure is more than 30 days away you can also place an order to put into storage. What type of sperm preparation do I need?

 

This depends on the type of procedure you will undergo (artificial insemination or in vitro fertilization) and what your physician/medical professional prefers. Before placing an order it is best to consult your physician regarding which sperm preparation to order.

Our IUI (pre-washed) and ICI (standard, unwashed) prepared specimens can be used for artificial insemination. Our IUI (pre-washed) specimens have been washed to remove the seminal plasma and are ready for an intrauterine insemination (IUI). Our ICI (unwashed) specimens can be used for an intracervical insemination (ICI) or can be prepared at your physician's office for an intrauterine insemination (IUI). The IVF as well as the ICI (unwashed) specimens can be used for in vitro fertilization with or without intracytoplasmic sperm injection and both preps will need further processing at your health care provider's office.

How many sperm vials do I need to order?


It is best to consult with your physician as the number you'll need may vary. You may wish to purchase multiple units for future cycles or for a second children by the same donor therefore place some units into storage with CLI.

I have selected a donor but am not ready to place my order. Can you hold units?

 

CLI offers a 7 business day courtesy hold with no obligation to purchase.  Please contact Client Services and request to place an order on hold.  You will need to contact Client Services by the 7th business day with your ship date or instructions to place the vial(s) into storage. Day one is the day you placed the order on hold. If we do not hear from you by 8:00 pm EST on the 7th business day, your hold order will automatically be canceled.

When will my order arrive via FedEx?

 

Fed Ex Priority Overnight shipping arrives the next business day, between the hours of 8:00AM and 12:30PM.

Fed Ex Standard Overnight arrives the next business day between the hours of 8:00AM and 5:00PM.

Fed Ex Two-Day shipping arrives the 2nd business day between the hours of 8:00AM and 5:00PM.

For example, if you need the samples to arrive at your physician's office on Friday and you select the Two-Day shipping then the samples would be shipped Wednesday of the same week. If you select Priority Overnight shipping the order would be shipped on Thursday to arrive Friday.

 When will my order arrive via local courier service?


If you live in the local Minneapolis/St. Paul area, your order will arrive at your health care provider's office by 2:30pm on the day of shipment.

Please be aware that the cutoff for same day local courier orders is 11:00 EST.

What type of information do you need to place an order?


You will need the following information available to place an order. You may need to consult with your health care professional to attain some of the items listed:

  • Patient name, address and phone number
  • Licensed Physician name and shipping address
  • Donor number
  • Number of units and preparation type (Pre-washed IUI, Standard or unwashed ICI, IVF)
  • Date the units need to arrive at the provider's office
  • Payment information

CLI accepts all major credit cards including Visa, MasterCard, American Express, and Discover. We have new financing options available. We also accept money orders and cashier checks; personal checks are not accepted. CLI must receive any money order or cashier's check before shipping date.  CLI does not work with or arrange payment with insurance companies, although we can provide you with copies of paid invoices to provide to your insurance company upon request.

What should I do once I place an order?


CLI suggests calling your health care provider with the delivery information once you have placed the order.

What if I need to return the specimen for storage?


We realize that situations may occur in which the return of the frozen specimens is necessary for storage purposes. In keeping with the AATB Standards, we will accept specimens returned to us providing (1) we are promptly notified that they are being returned for storage, (2) the specimens have not been removed from the shipping container and (3) the shipping container is received by CLI within 7 days from the initial shipment date. The return shipment for storage must be shipped via Overnight Service to comply with our 7 day time limit. Please contact our office promptly so that we are able to forward specific instructions for a safe return of your specimen. The returned specimens will be placed into storage for future use by the same patient. We cannot, and will not, use these specimens for any other physician or patient.

The specimens are also not able to be returned for any other purpose than storage; we do not and cannot accept specimens back for a refund once they have left our facility. You will be mailed storage and billing paperwork that you are required to complete and return to us before you are able to retrieve the specimens from your storage account for your next cycle.

What type of storage options do you offer if I want to purchasing and storing for future use?


Purchasing units to put into a storage account is a great way to ensure that vials from the donor you have selected are available for both current and future cycles. In addition, if you have purchased units that you eventually decide you do not want, we will buy them back at 50% what you paid CLI as long as the units have never left our facility.

How are the sperm specimens shipped?


All of our specimens are shipped in liquid nitrogen vapor tanks via FedEx or local courier. The liquid nitrogen vapor tank is inside of a cardboard container. The inner canister (vapor tank) is fastened using a cable tie to prevent tampering. Shipping papers attached to the outer case which contains a packing slip with the following information about your order: your health care provider's name and address, your name and address, your donor number and specimen preparation, number of vials, and post thaw count and motility for each units. Also included are a summary of records detailing the donor testing and a statement of donor eligibility, specimen thawing instructions, return instructions and prepaid return label.

Do you ship sperm specimens on dry ice?


No, CLI stores all of its specimens in liquid nitrogen, which has a temperature of -196 degrees centigrade. Dry ice has a temperature of -78.5 degrees centigrade that is far below the temperature in which we store specimens and can result in a partial thaw process that is detrimental to the quality of the samples.

How long are the specimens maintained frozen in the shipping tanks?


Our standard shipping tanks have a guarantee to keep specimens frozen for 7 days from the day of shipment (not arrival); day 1 is considered the day of shipment. Upon request and for an additional fee we can send your specimens in a 2 week tank in which the specimens are guaranteed to be kept frozen for 14 days from the date of shipment.

I'm not sure when my procedure is scheduled but want to go ahead and order donor sperm, what are my options?

Can CLI arrange payment of donor sperm with my insurance?

 

We do not work directly with insurance companies for reimbursement. We can provide you with copies of paid invoices to submit to insurance companies upon request.
